MEDICAL/DENTAL PLAN INFORMATION

During the past two decades health benefit plans have become an integral part of health care financial planning for many families. Benefits have given a "helping hand" to many receiving periodontal care. These plans are generally made available through employers, unions, trade groups, and associations. The scope of benefits varies greatly from one plan to the next and depends on several factors such as premium paid and cost containment provisions. Most dental plans have a yearly maximum, co-payment provisions, and deductibles. Our practice deals with hundreds of plans every year, all of which are different and constantly changing. So, please understand that we can provide general information about common coverage, but information regarding a specific plan is elusive.

Most dental benefit carriers have been approved by our accountants for assignment of benefits (the third party carrier pays us directly for a portion of your fee). As a courtesy to you we will submit claims to your approved carrier as services are provided.

At your request, a treatment plan fee can be submitted to your carrier for predetermination of benefits. This process usually takes from a few weeks to a few months. If you request this service, you may also schedule treatment prior to response if you wish. Predetermination of benefits is for your information and financial planning.

Treatment recommendations and fees are based on your particular needs only. You will find that our practice is not listed on any program's preferred provider (or discount) list. Dr. Gholson feels that is unfair to give one group of patients a discount and ask his other patients to pay an inflated fee to subsidize the discount. All of our patients should expect to be treated fairly and equitably.

Medicare

Periodontal procedures are not covered by Medicare.
